Volunteer Requirements
In order to volunteer on campus, Mt. Diablo Unified School District requires you to complete these steps. Thank you in advance!
We are incredibly grateful for the invaluable contributions made by parent volunteers to our PTA. Volunteers play a vital role in the success of our organization and the overall well-being of our school community. We extend our heartfelt appreciation to each and every parent volunteer for their boundless enthusiasm, time, and passion for making a difference in the lives of our students.

Please note: If you have been fingerprinted before, you do not need to do it again. TB clearance must be renewed once every four years.

2. Get Fingerprinted
Complete the Request for Livescan Service Application. You will need to bring your this Livescan form and valid form of photo I.D. to a Livescan vendor at your own expense. You can find a list of Livescan vendors here. To see if you qualify for a fee waiver please click here.
The PHE PTA sometimes offers opportunities to get fingerprinted on campus. If you are interested, please contact phepta94523@gmail.com.

3. TB Clearance
Contact a physician for a current Tuberculosis (TB) clearance. Please allow for the 48-hour reading requirement if a skin test is necessary. Proof of TB clearance cannot be older than 60 days from your fingerprint appointment. Please note, TB clearance must be renewed every four years.
In lieu of a skin TB test you may be eligible for a screening questionnaire. A volunteer may be able to do the screening for you for free. Email PHE PTA (phepta94523@gmail.com) to set it up.

4. Turn in Your Documentation
Please return the completed Livescan form, Volunteer Request form, and valid TB clearance via email to personnelfrontdesk@mdusd.org. Completed forms can also be faxed to (925) 676-4092, sent by US Mail or delivered to our district office located at 1936 Carlotta Drive, Concord, CA 94519.

You will not be added to our master list of school volunteers until ALL required documents have been returned to the district office, and your fingerprints have been cleared by the Department of Justice, which can take 1-30 days.
5. PHE Orientation
